Main Street Med Spa Company Policies

Effective September 1, 2022


Main Street Med Spa reserves the right to change and implement policies and charge fees as listed below. Prices, services, and policies are subject to change.


Roderick Warren, MD, Main Street Med Spa, of Main Street Med Spa LLC, is a registered and licensed legal entity in the state of Kansas. Main Street Med Spa follows all legal guidelines set forth by local, state and federal law. Main Street Med Spa, and all contracted providers/LLCs are in compliance with their applicable boards including but not limited to; the Kansas State Medical Board, Board of Cosmetology, Massage/Bodywork Panel, and the Federation of State Massage Therapy Board.


Main Street Med Spa, LLC has a legally binding agreement with Roderick Warren, MD of R. Warren, MD, LLC enlisting him/her as supervising physician. All contractors and service providers have legally binding agreements on file with Main Street Med Spa, LLC. Any and all contractors providing nay service deemed medical by the state of Kansas have a legally binding agreement with Roderick Warren, MD of R. Warren, MD, LLC enlisting him/her as supervising physician. The supervising physician has authorized all providers to perform medical services where applicable by law based on their certifications, training and licensing.


Main Street Med Spa, LLC policies, procedures, and protocols are developed, authorized, and established to follow all guidelines, requirements and laws set forth by the owner, supervising physician, product/device manufacturers, and state requirements. All providers are knowledgeable and practice all safety and emergency protocols in compliance with the local, state, and federal laws. 


Client Responsibility 


The client is responsible for the payment of all services rendered. Payment is expected at the time of service for all charges towards the current visit, as well as previous balances (i.e. late fees, no-show fees). We accept cash, Visa, Master Card, American Express, Discover, Apple Pay and Cherry Payment Plans. Gratuity for services is never expected but always appreciated.


Card on File Policy


All appointments require a debit or credit card on file. If the Last Minute Notice/ No notice occurs, the card will be charged. Main Street Med Spa LLC, its employees, and contractors do not have access to this information once it has been added to our system. All card information is kept on a secure server through our Payment Operating System. You may see a small charge of 0.50-$1.00 as a temporary hold to ensure the card is active and has valuable funds. The charge will be reimbursed automatically.


Deposit Policy


Main Street Med Spa reserves the right to implement appointment deposits. Some services may require a deposit at the time of booking your appointment. Appointment deposits are non-refundable and serve to reserve our medical staff and our equipment specifically for you and your treatment. Any changes or cancellations to these appointments without the required amount of notice will forfeit the deposit in its entirety and you may be subject to additional fees. Appointments may be changed without penalty if the minimum amount of notice is provided, per the table below. If you wish to include the deposit into a Cherry payment plan, you must be approved for the total amount needed and finalize your payment plan before you may book your appointment.


Last Minute Notice/ No Notice Fee


If you cancel or reschedule within 24 hours of your appointment, fail to attend, or are more than 30 minutes late, you will be automatically charged a no-show fee of $35 on the card you keep on file. This fee may or may not be applied towards the cost of your rescheduled appointment at the discretion of the affected provider. This policy is in place to respect the time of our providers and our clients. Cancellations with less than 24 hours' notice are difficult to fill. By giving last minute notice or no notice at all, you prevent someone else from being able to schedule that time slot and leave a vacancy in your provider's schedule. If you break this policy on two or more occasions, you will be required to request an appointment, either in person or by phone, rather than book directly online or in person, and you may be required to pay for your next service in full at the time of booking. 


Late Policy


If you arrive at your appointment late, you may have to reschedule due to time constraints. Arriving late to your appointment may reduce your total appointment time. For time based appointments, you will still be charged for the full appointment time. For example, if you arrive 10 minutes late to your 60 minute massage, your appointment length may be reduced to 50 minutes. You will still be charged for the 60 minute massage. If you are more than 30 minutes late to your appointment you will be charged a $35 Last Minute Notice/No Notice Fee.


Refund Policy


Your complete satisfaction is our first priority. In the unlikely event you are at all unsatisfied with your purchase, please let us know immediately. We accept returns for unopened and unused merchandise 7 days after purchase. Treatments and procedures are non-refundable. If you do not wish to complete your pre-paid service, we can give you a credit with Main Street Med Spa that can be used towards another service or product.




Multiple Session Services


We may recommend multiple sessions for certain services to ensure optimal results. Sessions can be paid for individually or you may purchase our predetermined bundles of multiple sessions. Bundles must be paid for in full, at the time of the first session. 


Right to Refuse


Main Street Med Spa reserves the right to refuse service (depends on your state). Main Street Med Spa will not discriminate against race or color, national origin or citizenship status, religion or creed, sex, age, disability, pregnancy, genetic information or veteran status. Certain services cannot be performed on everyone due to health risks and contraindications. 


HIPAA Compliance


Main Street Med Spa and all employees, contracted individuals, and service providers will abide by all HIPAA policies and set forth actions and policies to prioritize client confidentiality.


Media Policy 


Photographic documentation will be taken. As part of your treatment, we will be photographing the treatment area of your body/face (and in some cases, filming the treatment process). This will allow us to visually monitor your individual progress and see the results of your treatment over time. We reserve the right to publish any content that does not reveal your identity. If any of the content is in question or does reveal your identity, we will always ask for your permission first. We would appreciate your willingness to share your outcomes and results with others, for both training and marketing purposes within the beauty, cosmetic and aesthetic industries. In all cases, we will do everything we can to keep your identity anonymous.
